Dear Parents:
Welcome (or welcome back!) to St. Rose of Lima School! Per the state, there are certain requirements that the nurses’ office will need to have ON or BEFORE the first day of school for all Kindergarten students, transfer students, and all students entering sixth grade. In addition, there are forms that are needed from each family, each year, if it is applicable to your students.
Please read all sections below carefully and download the appropriate forms from the links highlighted in blue. Make sure all forms are complete and signed by both the physician and parent where indicated. Please pay special attention if your child requires medication in school, has an epi-pen, or has asthma, as there are specific forms which MUST be filled out by both the parent and the physician each new school year. Also, the medication must be sent into school in its original container by the FIRST DAY of school.

Allergy & Asthma Forms
All new and current families: Please see below for information and forms that are needed each school year from all students if it is applicable to them. For more information about our food allergy precautions please click here.
- Medication Administration Form: If your child will need to take medication in the nurses’ office during the school day, we need a new order on file each school year. Please click here for the medication form instructions.
- Epi-Pen Forms Overview: If your child has a severe allergy and requires an Epi-pen, multiple forms are required to be filled out and signed by you and your physician.
- Asthma Requirements Overview: If your child has asthma, multiple forms are required.
Sports Physical Forms (Grades 6, 7,8)
Students in grades 6, 7, & 8 are required to have a physical and clearance from a doctor before playing sports. Parents of student athletes in grades 6, 7 & 8 are required to read and complete the Sports Physical Forms and information here before their student(s) will be allowed to participate in school-related sports practices & competitions.
